Milo is bringing an unusual twist to horror survival, combining battles against cultists with rescuing rats and escaping to an arcade. The upcoming PC shooter is scheduled to enter Steam Early Access on 29 October 2026.
Developed by rat cream games with publishing and co-development support from Yarrawah Interactive, Milo takes place in a nightmare shaped by World War era imagery. Yarrawah describes its approach as combining Call of Duty style combat with progression inspired by Risk of Rain 2.
Players awaken in an unfamiliar body and are forced to take part in Milo’s cruel game. They can face its endless cultist waves alone or join an online group of up to four players.
Rats sit at the centre of the survival loop. Fallen enemies feed the growing group and charge an ability to pop corpses. Staying longer offers more opportunities to rescue rats, but also increases what players risk losing before they escape.
Extracting with the rats takes players to Traang, Milo’s arcade. Activities including basketball provide tickets to spend on upgrades, connecting the fighting to another part of the game’s progression.
The arsenal includes weapons such as the M1 Garand, Kar98 and PPSh-41. Yarrawah’s work on the project covers weapon models and animations, redesigned characters and a move to Unreal Engine 5.5.

Stackable buffs let players shape their builds, while the bizarre Cream Machine can further alter a run. The current Steam listing specifies that Early Access will launch with one playable class, Survivor.
In an August development update, creator Spencer explained that the game had changed substantially over the preceding months. He said performance had improved and mechanics had evolved, bringing the experience closer to his original vision.
That update also confirmed the removal of the existing demo. Spencer said it no longer represented the game accurately and that maintaining it alongside development had become too demanding.
The developer currently expects Early Access to last approximately two years, although that estimate could change. Plans for the full release include additional maps, weapons, enemies, arcade content and rat customisation.
